Important To Note: This survey is unique to California and the data collected will be provided to the State of California’s Office of Statewide Health Planning and Development (CAPA will not provide them with any individual identifying information). The questions on the survey do not relate to salary or prescribing activity. Just a few quick, multiple choice questions about where you work and live and a bit about your work setting and patient population. The last time the State collected this data for PAs was 14 years ago. Your response is imperative to make the State of California report on PA Practice valid and meaningful.
