Job Details – 

• Position Type: Temp-to-Perm – Advanced Practice Provider / Nurse Practitioner
• Location: Fresno, California 
• Specialties: Primary Care/Nephrology
• Schedule: Full-time, Monday – Friday (no weekends, no on-call)
• Shifts per Week: 5
• Shift Duration: 8-hour days
• Hours per Week: 40
• Contract Duration: 26 weeks
• Estimated Weekly Pay: $3,840 – $4,550
• Setting: Hybrid – 3-4 days in-home patient visits, 1-2 days remote (admin/charting/telehealth)

Qualifications – 

• Minimum 2 years of experience as a Nurse Practitioner or Advanced Practice Provider
• Experience in home health, primary care, internal medicine, or clinical support preferred
• Experience managing care plans is a plus
• Not suitable for urgent care, hospitalist, surgical, or pediatric backgrounds
• Active California NP license required
• Strong communication and organizational skills
• Ability to travel within assigned Fresno, CA territory as needed

Responsibilities – 

• Conduct initial patient assessments, follow-ups, and hospital transitions for chronic and complex conditions
• Perform comprehensive, hands-on assessments and full vitals
• Educate patients and families on disease processes and care plans
• Manage medication reviews and therapy management
• Visit 4-5 patients per day
• New patient visits: 90 minutes; re-visits: 30-60 minutes
• Participate in POD rounding to discuss high-risk patients and care updates with the healthcare team
• Complete documentation using Athena and Salesforce platforms
• Utilize AI-powered scheduling tools to optimize patient visits and clinical workflow
