Role Overview

Location: San Franciso, CA
Flexible full-time 4-5 day/week schedule Monday through Friday
15-18 patients per day
Post-Acute care inpatient setting
No call

What You'll Do

Deliver comprehensive primary care
Manage acute and chronic conditions
Develop personalized care plans
Integrate and work closely with the interdisciplinary team
Partner and work collaboratively with a TeamHealth physician

What We Are Looking For

Experienced post-acute care clinicians
FNP, CNP, AGNP, APRN, or PA
Active California NP/PA license
Active DEA

Our compensation is a productivity-based model (fee for service); expected compensation is estimated salary range of $171,000 to $206,000 annually with no cap on productivity income potential with full benefits.
